Reference page for campaign reviewers. This page demonstrates the SMS opt-in web form a care circle contact uses: mobile phone number, consent disclosure, and preference / opt-out controls. Real contacts receive a unique preference link by text after a facility enrolls them; this demo is not connected to any contact record and saves nothing.

Care circle SMS opt-in & preferences (demo)

Care Elevate sends transactional care circle notifications — care updates and new message alerts — to family members and care contacts enrolled by their facility. No marketing. SMS is optional and unchecked by default. Message bodies never contain health information; they link to the secure care portal.

SMS notifications are optional. You can use care circle preferences and email updates without agreeing to text messages. Leave the SMS box unchecked to decline texts.

You can also opt out at any time by replying STOP to any message — it is honored immediately.

How enrollment and opt-in work

  1. A facility administrator enrolls the contact (name, relationship, mobile number) during resident intake or via the admin dashboard — there is no public sign-up
  2. The contact's first text identifies Care Elevate, describes the message types, notes that message frequency varies and message & data rates may apply, and includes STOP/HELP keywords
  3. That first text also carries the contact's unique preference link, which opens a page like this one. The contact enters or confirms their mobile number, gives explicit SMS consent, adjusts delivery preferences, or opts out permanently
  4. Consent and opt-out choices are stored with a timestamp on the contact record
